Membership Application Form
To become a member of Moreton Hall Health Club, please follow the steps below:
- The first step is to fill in the form below, selecting the membership(s) you would like and the personal details of the member(s)
- Submit the Form and click "Print" to print off the following page.
- Fill in the relevent payment details on the printout.
- Mail or Fax the printout to us and you will be notified when your payment has been recieved and will be sent a Membership Package.

All categories enjoy free useof the Sauna and Steam. Individual, Couples and Family membership includes free use of Laydown Sunbed. Upright Tanning booth £5 per session.Members children aged fron 2-18 years can be added to any category for £8.50 per month (gym 16+). Family membership includes childen up their 18th birthday or until they have completed their education.
